• Our Dental Savings Club is not a qualified health plan under the Affordable Care Act.
• Your payments must be current to receive the services and discounts included in the membership plan.
• Payment for treatment not included in the membership plan is due at the time of service.
• DDS Family Dental Savings Club benefits may not be combined with insurance or any other offers or discounts.
• Our Savings Club is for services provided by Dr. Daniel D. Seikel DDS and does not cover procedures completed by specialists outside our practice.
• Our Dental Savings Club is annually renewable, fees are nonrefundable and nontransferable. No substitutions are allowed. All benefits must be used within a 12 month period.
